Your loved one must go through a licensed Hospice Agency. The two we usually work with in Monroe County are UR Medicine Homecare/VNS and Lifetime Care Hospice. After talking to the Hospice Team Nurse and Social Worker, you will be guided through the process.
Once your loved one has been admitted to our house, the agency will continue to oversee the Hospice Plan of Care along with the attending physician. Advent House will then provide the excellent hands-on care and support. We are not responsible for medical decisions but will collaborate with the agency and physician to provide the best possible care for your loved one.
